Apply by Mail
If you are able to legally document your name change and you are not a minor, you can apply for your name change passport by mail. If your most recent passport was issued less than a year ago, there will be no charge for your new passport unless you request expedited service. The government charges a $60 fee for expedited requests.

Apply in Person
If you cannot legally document your name change, you must apply in person at a passport acceptance facility or passport agency to get your new passport.
To apply in person, follow the steps listed below: Complete Form DS-11: Application for a U.S. Passport, but do not sign it. Take the completed, unsigned form and all supporting documentation to a passport acceptance facility or passport agency in your area. An acceptance agent will instruct you to sign it when appropriate.
You must provide the following when applying for your name change:Your current, valid passportApplicable fees
At least three public records that show:your birthdate where you were bornexclusive use of your assumed name for at least 5 yearsPassport photo

How Long Will it Take to Issue My Passport?
The average processing time for routine service is four to six weeks, and the average processing time for expedited service is two to three weeks. However, it can take longer during peak application times throughout the year. Generally, the U.S. Department of State receives the highest volume of applications from January-July in any given year.
Can I Get it Faster?
If you have a trip planned and you can’t wait the two to six weeks it will take the government to issue your name change passport, you have two choices. You can apply at a passport agency. To apply at one of these facilities, you must make an appointment. When you arrive for your appointment, a representative will take your application and the supporting documentation required to issue your passport. Usually, you will have your passport the same day, but there are no guarantees.
In addition, there are only 24 passport agencies in the entire country, most of which are located in large cities. If you don’t live near one, it may not be realistic for you to apply at a passport agency. So, what should you do?
