Request New Pages Using Form DS-4085

Apply for a New Passport Book

Applying for a new passport book is recommended for U.S. citizens who travel frequently. When you apply for a new book, do not use form DS-4085. Instead use form DS-82 if you are eligible to renew your passport by mail. Send the form along with a written request to add pages to passport, your current passport, a passport photo and the appropriate fees to the address on the form.

If you are not eligible to renew your passport by mail, you cannot use form DS-82. Instead you must use form DS-11 and apply for a new book in person at a passport acceptance facility or passport agency. U.S. passport applications are accepted by appointment only at passport agencies. Make sure you call ahead to schedule your appointment.
